Enhance Referrals and Follow-up Services

What Can Policymakers Do?

· Use Medicaid and SCHIP to provide care coordination services to families. Connecticut’s Help Me Grow program involves both a community health liaison who identifies community resources and a family care coordinator who identifies concerns, helps schedule appointments, and reports back to the primary care provider.

· Support primary care practices that incorporate social and developmental services . North Carolina expanded its Medicaid rules to allow billing by physicians for on-site, supervised mental health services provided by employees who are either clinical social workers or clinical nurse specialists with psychiatric training.
